Immunization Requirements for All Students: Due on or before the first day of school.
- 4 doses of DTaP (one dose on or after 4th birthday)
- 4 doses of Polio (4th dose on or after 4th birthday and 6 mos after previous dose)
- 2 doses of MMR
- 3 doses of Hepatitis B
- 2 doses of Varicella
- 1 dose of Tdap (Tetanus, Diphtheria, acellular Pertussis) grades 7-12
- 2 doses of MCV (Meningococcal Conjugate Vaccine) with first dose being in 7th grade and 2nd dose by 12th grade

Students requiring any medication during school hours are required to have a completed medication form. This form must be signed by both the physician as well as the parent in order for the Nurse to administer medication. Prescription medicines must be brought to school in a pharmacy labeled bottle, which contains instructions on how and when the medication is to be given. Medication orders are also required for any over-the-counter medications.
Health care plans are to be completed in collaboration with parents and their health care provider for any student with diabetes, seizures, food or bee sting allergies, asthma, or any other health concern that may impact their learning at school. Should a student require a medication while at school, the appropriate paperwork must be completed as per school policy.
